At the receptor level, tirzepatide activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. Both belong to the class B family of G protein-coupled receptors and signal largely through cyclic AMP accumulation. The compound binds the two receptors with differing affinity, and the pattern of signaling at each site is described in the literature as biased rather than simply proportional to occupancy. Tissues carrying these receptors include pancreatic islets, adipose tissue, the central nervous system, and the gastrointestinal tract. The relative weight of each receptor population in producing metabolic effects continues to be studied.
Published work supports the view that engaging two incretin receptors produces changes in glucose handling and body weight larger than those seen with single-receptor activation. Why that difference arises is not fully settled. Open questions include how much of the observed weight effect depends on central versus peripheral signaling, and whether the two receptors form interacting complexes. Most reported findings come from controlled trials and animal models, and translation between species is imperfect. Further research is expected to refine these points over time.
Routine characterization of the peptide relies on reversed-phase high-performance liquid chromatography for purity assessment, usually with ultraviolet detection near 214 nanometers. Intact mass measurement by liquid chromatography coupled to mass spectrometry confirms molecular identity against a theoretical value. Sequence-level confirmation uses enzymatic digestion followed by tandem mass spectrometry, an approach known as peptide mapping. Amino acid analysis gives an independent check on composition. Circular dichroism spectra are used to estimate helical content in aqueous buffer.
Stability depends strongly on physical form. The dry powder is generally regarded as stable for extended periods when held at or below minus twenty degrees Celsius in a sealed, desiccated container. In solution, degradation pathways include deamidation of asparagine and glutamine residues, oxidation of methionine, and aggregation. Reaction rates for these pathways rise with temperature. Repeated freezing and thawing of solutions promotes aggregation, and light exposure can accelerate some oxidative changes. Buffer composition and pH influence which pathway dominates at a given temperature.

Structural work on the molecule centers on a C20 fatty diacid moiety attached through a linker to the peptide backbone. This side chain promotes reversible binding to serum albumin, which slows renal clearance and supports a prolonged action profile. The peptide backbone incorporates aminoisobutyric acid substitutions that limit recognition by digestive enzymes. Together these modifications produce a molecule that is stable enough for subcutaneous delivery but still dependent on careful manufacturing control. Analytical characterization of the active pharmaceutical ingredient typically follows the conventions used for other synthetic peptides.
Tirzepatide is a synthetic peptide composed of 39 amino acids. It acts as a dual agonist at two incretin receptors, the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. The molecule was designed by modifying the native sequence of glucose-dependent insulinotropic polypeptide to improve metabolic stability and extend its circulation time. Its structure includes several non-natural amino acid residues and a fatty acid side chain. These features distinguish it from earlier single-receptor incretin analogs studied in the same period.

==== Dopaminergic pathway and reward suppression ==== The mesolimbic dopaminergic circuit functions as a substrate for KOR-regulated mood homeostasis. Dynorphin is synthesized and released by dopamine D1 receptor-expressing medium spiny neurons within the NAcc, establishing a local negative feedback loop that suppresses dopamine release. KOR activation on dopamine terminals inhibits dopamine release through multiple mechanisms: increased potassium conductance via G protein-coupled inward-rectifier potassium (GIRK) channels, suppression of calcium entry, activation of protein kinase C-β (PKCβ), c-Jun N-terminal kinase (JNK), and ERK, as well as facilitation of dopamine transporter (DAT) function through ERK1/2-dependent pathways that accelerate dopamine reuptake. Additionally, KOR activation on local dynorphin-expressing neurons produces presynaptic inhibition of both glutamatergic and GABAergic afferents onto D1 receptor-expressing medium spiny neurons, with preferential suppression of amygdala inputs to D1-MSNs while facilitating integration of hippocampal/amygdalar inputs onto D2 receptor-expressing neurons through disinhibition. In the caudal NAcc shell, KOR-induced dopamine suppression triggers anxiogenic behaviors accompanied by reduced locomotor activity. Conversely, in the rostral shell, KOR activation produces attenuated dopaminergic suppression with diminished aversive behavioral consequences.
== Overdose == Little information is available about clinically significant selegiline overdose. The drug has been studied clinically at doses as high as 60 mg/day orally, 10 mg/day as an ODT, and 12 mg/24 hours as a transdermal patch. In addition, deprenyl (the racemic form) has been clinically studied orally at doses as large as 100 mg/day. During clinical development of oral selegiline, some individuals who were exposed to doses of 600 mg developed severe hypotension and psychomotor agitation. Overdose may result in non-selective inhibition of both MAO-A and MAO-B and may be similar to overdose of other non-selective monoamine oxidase inhibitors (MAOIs) like phenelzine, isocarboxazid, and tranylcypromine. Serotonin syndrome, hypertensive crisis, and/or death may occur with overdose. No specific antidote to selegiline overdose is available.

Many drugs are mimics or prodrugs of natural nitrogen-containing signal molecules: for example, the organic nitrates nitroglycerin and nitroprusside control blood pressure by metabolising into nitric oxide. Many notable nitrogen-containing drugs, such as the natural caffeine and morphine or the synthetic amphetamines, act on receptors of animal neurotransmitters.
A codon table can be used to translate a genetic code into a sequence of amino acids. The standard genetic code is traditionally represented as an RNA codon table, because when proteins are made in a cell by ribosomes, it is messenger RNA (mRNA) that directs protein synthesis. The mRNA sequence is determined by the sequence of genomic DNA. In this context, the standard genetic code is referred to as 'translation table 1' among other tables. It can also be represented in a DNA codon table. The DNA codons in such tables occur on the sense DNA strand and are arranged in a 5′-to-3′ direction. Different tables with alternate codons are used depending on the source of the genetic code, such as from a cell nucleus, mitochondrion, plastid, or hydrogenosome. There are 64 different codons in the genetic code and the below tables; most specify an amino acid. Three sequences, UAG, UGA, and UAA, known as stop codons, do not code for an amino acid but instead signal the release of the nascent polypeptide from the ribosome. In the standard code, the sequence AUG—read as methionine—can serve as a start codon and, along with sequences such as an initiation factor, initiates translation. In rare instances, start codons in the standard code may also include GUG or UUG; these codons normally represent valine and leucine, respectively, but as start codons they are translated as methionine or formylmethionine.

=== 2016: Court of Arbitration for Sport verdict === On 12 January 2016, CAS handed down a guilty verdict on the thirty-four Essendon players, overturning the AFL Anti-Doping Tribunal's not-guilty verdict, after finding it was comfortably satisfied that the players were injected with Thymosin beta-4. Key to the appeal's success was the treatment of evidence: the CAS rejected the AFL Tribunal's approach, known as "links in the chain", where any individual chain of evidence is dismissed if any one link within it cannot be proven, and endorsed WADA's approach, known as "strands in the cable", where individual evidence chains with missing links may still be accepted if the combination of all such chains forms a sufficiently strong case. A complete account of the verdict and the arguments made by each side was released publicly. Vital to the case was determining whether or not the unspecified Thymosin used in the program was the banned Thymosin Beta-4 or a different, legal variety of Thymosin. A paper trail had confirmed that Dank had been dispensed Thymosin Beta-4 by the Como Compounding Pharmacy; however, no direct evidence was found that it was this Thymosin rather than a different legal Thymosin which had been administered to players, and this missing evidence link had been key to the AFL Tribunal's not guilty verdict under the 'links in the chain' method.

== Epidemiology == Tendon injury and resulting tendinopathy are responsible for up to 30% of consultations to sports doctors and other musculoskeletal health providers. Tendinopathy is most often seen in tendons of athletes either before or after an injury but is becoming more common in non-athletes and sedentary populations. For example, the majority of patients with Achilles tendinopathy in a general population-based study did not associate their condition with a sporting activity. In another study the population incidence of Achilles tendinopathy increased sixfold from 1979–1986 to 1987–1994. The incidence of rotator cuff tendinopathy ranges from 0.3% to 5.5% and annual prevalence from 0.5% to 7.4%.
=== Fats, minerals and proteins === Are produced when collagen proteins are manufactured. The main areas of application for Fat, Mineral and Protein Ingredients are human food, pet food and animal feed. The fats, for example, serve as energy sources. Proteins can improve the texture and hence the enjoyability and digestibility of foodstuffs and animal feed. Further applications are in technical areas and are used in the manufacture of bone china, construction molds, lubricants for machines and anti-rust agents.
=== Gender === Women typically make up some 80% of autoimmune disease patients. Whilst many proposals have been made for the cause of this high weighting, no clear explanation is available. A possible role for hormonal factors has been suggested. For example, some autoimmune diseases tend to flare during pregnancy (possibly as an evolutionary mechanism to increase health protection for the child), when hormone levels are high, and improve after menopause, when hormone levels decrease. Women may also naturally have autoimmune disease trigger events in puberty and pregnancy. Under-reporting by men may also be a factor, as men may interact less with the health system than women.

It is a synthetic peptide and a dual agonist of two incretin receptors. It is not a small molecule, and it is not structurally related to the older single-receptor peptide agonists.
The C20 fatty diacid promotes tight binding to serum albumin. That binding reduces renal clearance and extends circulation time compared with an unmodified peptide of similar length.
It is not fully established. Studies indicate that both receptors contribute to the observed effects, but the exact split between the two signaling pathways in humans remains an open question.
Peptide mapping with tandem mass spectrometry is the standard approach. The peptide is digested with an enzyme such as trypsin, and the resulting fragments are matched against the expected sequence.
